Responsibilities:- Provide hands-on nursing care to patients with physical disabilities and chronic illnesses.
- Participate in patient and family educational activities, implementing individualized teaching and discharge plans.
- Perform clinical tasks, reinforcing care provided by other team members and demonstrating clinical judgment.
- Coordinate nursing care activities with the interdisciplinary rehabilitation team to achieve overall goals.
- Communicate effectively with patients, families, and healthcare team members, ensuring legal documentation and reimbursement requirements are met.
- Successful completion of a Licensed Practical Nurse program from an accredited college.
- Experience in Post-Acute Care or an Acute Care environment.
- Current licensure in good standing to practice as a Licensed Practical Nurse within the state you are practicing.
- Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
