Nurse Care Manager

LHC Group is seeking a skilled RN Care Manager to join our team in Redding, California. As a key member of our home health care team, you will be responsible for providing high-quality patient care and coordinating clinical services to ensure the best possible outcomes for our patients.
About the Role:- Provide clinical services to patients in their homes, as prescribed by physicians and in accordance with applicable laws and regulations.
- Coordinate and supervise an interdisciplinary team of staff to ensure continuity of care and high-quality patient outcomes.
- Develop and implement individualized patient plans of care, in collaboration with physicians and other healthcare professionals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plots start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Current RN licensure in state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- CA: One year prior professional nursing experience
