Addiction Medicine Advanced Practitioner Training Program

The Addiction Medicine Advanced Practitioner Training Program provides integrated mental health and substance use disorder (SUD/OUD) services in a primary care underserved community-based setting. This comprehensive training program aims to foster robust clinical skills and augment expertise among clinicians who will provide mental health and addiction prevention, treatment, and recovery.
About the Role:
This trainee position offers a unique opportunity to participate in Medications for Opioid Use Disorder (MOUD) in community-based settings, alongside additional clinical training beyond the didactic component. As a member of our team, you will collaborate with caregivers inside and outside of the immediate care team, connect patients to social supports, and ensure patient retention in care.
Responsibilities:
• Participate in MOUD in community-based settings and additional clinical training to enhance clinical skills
• Connect and manage patients receiving MOUD, linking them to behavioral health counseling and social supports
• Collaborate with care givers inside and outside the immediate care team to ensure seamless patient care
• Demonstrate effective communication with patients and the immediate care team
• Conduct initial and ongoing assessments of patient medical, physical, and psychosocial status
• Perform history and physical examinations, develop assessment and treatment plans, and participate in clinical patient care
• Record progress notes, order tests, examinations, medications, and therapies as necessary
• Arrange for discharge and aftercare, write and dictate admission notes, progress notes, procedure notes, and discharge summaries
• Provide patient education and counseling on health status, test results, disease processes, and discharge planning
• Perform procedures as required
Requirements:
To be considered for this role, you must have a Master's Degree in Nursing Practice (Nurse Practitioner) and certification(s) and license(s): Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Certified Nurse Practitioner - American Academy of Nurse Practitioners.
Estimated Salary:
$120,000 per year, depending on experience.
