Lead Advanced Care Clinician in Scranton, PA

Geisinger Community Medical Center (GCMC) is seeking a highly skilled Nurse Practitioner or Physician Assistant Hospital Medicine to join our team. As an advanced care clinician, you will be responsible for delivering high-quality patient-centered care to residents throughout Lackawanna County and northeastern Pennsylvania.

Responsibilities
As a Lead Advanced Care Clinician, your primary responsibilities will include:
- Evaluating patients, assessing their needs, and developing treatment plans.
- Making accurate and complete entries into the electronic medical record, ordering and interpreting diagnostic testing, and prescribing appropriate medical therapy.
- Communicating effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Providing education and support to patients and families to promote health and wellness.
- Collaborating with multidisciplinary teams to achieve excellent patient outcomes.
Requirements
To be successful in this role, you will need:
- A Master's Degree in Nursing or a related field.
- Current licensure as a Nurse Practitioner or Physician Assistant in Pennsylvania.
- Basic Life Support Certification and certification as a Nurse Practitioner or Physician Assistant.
- Experience working in an acute care setting, preferably in a hospital medicine environment.
