Job Summary

We are seeking a highly skilled Medication Reconciliation Technician to join our pharmacy team at Penn State Health Lancaster Medical Center. As an integral member of our team, you will work closely with newly admitted patients in the emergency department to obtain and document an accurate medication list, helping to prevent medication duplications, omissions, and errors.

The information obtained will be entered into the electronic health record to be used by physicians and other healthcare providers. You will use this information to determine medication orders during hospital admission and upon discharge from the hospital.

This role requires knowledge of pharmaceutical terminology, metric systems, and computer systems, as well as excellent customer service skills and the ability to effectively work and communicate with patients, staff, and outside providers.

Responsibilities
  • Obtain and document accurate medication lists for newly admitted patients
  • Enter information into the electronic health record
  • Determine medication orders during hospital admission and upon discharge
  • Communicate effectively with patients, staff, and outside providers
Requirements
  • High School Diploma or equivalent required
  • Current Pharmacy Technician certification or licensed practical nurse by state of employment
  • Minimum three years of medication history or pharmacy experience preferred
