Job Summary:
Veterans Health Administration is seeking a highly skilled and dedicated Primary Care Physician to join our team in Peoria, IL. This is an exciting opportunity for a physician with excellent clinical skills and a passion for providing high-quality patient care. As a member of our healthcare team, you will be responsible for delivering comprehensive medical care to a panel of patients under the Patient Aligned Care Team (PACT) model.

About the Job:
The successful candidate will have a strong background in family medicine or internal medicine and experience working in an interdisciplinary team environment. They will be expected to manage chronic conditions, evaluate and treat inter-current medical conditions, and collaborate with a PACT to provide accessible, comprehensive, coordinated, continuous care to assigned patients. Additionally, they will perform disease prevention related activities, including screenings and health promotion activities, and refer patients to mental health providers as needed. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work well in a fast-paced environment.

Responsibilities:
* Manage chronic conditions and evaluate/treat inter-current medical conditions* Collaborate with PACT to provide accessible, comprehensive, coordinated, continuous care to assigned patients* Perform disease prevention related activities, including screenings and health promotion activities* Refer patients to mental health providers as needed* Supervise/coordinate with primary care mid-level providers as necessary* Participate in peer review of other primary care providers through periodic chart reviews* Act as surrogate for absent primary care providers when needed* Attend regular primary care staff meetings, departmental meetings, and medical staff meetings* Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy* All administrative work should be completed in a timely fashion* Clinical reminders should be addressed as medically appropriate* Quality of care should meet professional standards at all times

Requirements:
* United States Citizenship* Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine* Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia* Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification* Proficiency in spoken and written English* Board Certification in Family Medicine or Internal Medicine preferred* Experience working in an interdisciplinary team environment preferred* Strong background in family medicine or internal medicine* Excellent clinical skills and a passion for providing high-quality patient care* Ability to work well in a fast-paced environment* Excellent communication and interpersonal skills*
