Job Overview

The Camden Center is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in supporting our clients and their mentors/companions in achieving their goals and navigating the case management program.

Key Responsibilities
  • Develop and implement individualized client reintegration plans, working closely with clients, mentors/companions, and other stakeholders to ensure successful outcomes.
  • Provide regular communication and support to clients, mentors/companions, and other team members, ensuring a seamless and collaborative approach to care.
  • Manage client information, including toxicology screens and medication management, in accordance with program guidelines and regulations.
  • Collaborate with mentors/companions to provide phone support and supervisory communication, ensuring a strong and supportive relationship.
  • Document all interactions and discussions with team members, clients, and mentors/companions, maintaining accurate and up-to-date records.
Requirements

To be successful in this role, you will possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, mentors/companions, and other stakeholders. You will also have a strong understanding of case management principles and practices, with the ability to apply this knowledge in a dynamic and fast-paced environment.
