Job Title: Licensed Practical Nurse (LPN)

Altru Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Job Summary:

The LPN will be responsible for performing and documenting assessments, ensuring health history and admission processes are completed, and providing patient care. The ideal candidate will have excellent communication skills, a strong work ethic, and the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Assist registered nurses and other licensed practitioners in identifying patient needs, priorities of care, and goals.
  • Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
  • Modify the plan of care in collaboration with licensed practitioners.
  • Perform other duties as assigned to meet the needs of the department/organization.
  • Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
  • Assign or delegate the responsibility for performance of nursing interventions to other persons for which that person has the necessary skills and competence to accomplish safely.
  • Identify the early warning signs of a change in a patient's condition and report appropriately.
  • Implement nursing interventions and prescribed medical regimens in a timely and safe manner and document nursing care provided accurately and timely.
  • Collaborate and communicate relevant and timely patient information with patients/families and other multidisciplinary team members to ensure quality and continuity of care.
  • Report the patient's self-report/symptoms of pain promptly according to standard operating procedure.
  • Participate in the health education approved by a licensed practitioner.
  • Assist in the discharge planning, appropriate education, and counseling based on identified needs to optimize patient/family understanding.
Certifications:
  • Licensed Practical Nurse | Minnesota Board of Nursing | Prior to Start Date | HR Primary Sources
  • Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 days of Start Date | Learning Management System
Working Conditions:

This position requires the ability to work in a fast-paced environment and to adapt to changing situations. The LPN will be required to stand, walk, and move frequently throughout the workday. The position also requires the ability to lift, carry, and push/pull medium-weight objects.

Driving Requirement:

This position is classified as an Infrequent Driver.
