Job Summary

Polara Health is seeking a skilled Adult Community Care Specialist to provide comprehensive care and support services to adults with multifaceted needs. As a key member of our team, you will work collaboratively with clients, families, and other healthcare professionals to navigate and meet the complex needs of our clients.

Key Responsibilities
  • Provide regular and consistent services and supports to clients identified appropriate for community supports
  • Manage all aspects of clinical treatment, including referrals to additional resources, crisis assessment, and support as needed
  • Work with clients on skill building, activities that support stabilization in living environments, connection to community resources, and natural supports
  • Service provision shall be conducted primarily in the client's living and other environments as necessary or appropriate
  • Provide outreach and engagement when needed
  • Closely collaborate with other care teams within Polara Health to coordinate and meet client needs
  • Facilitate Adult Recovery Team Meetings (ARTs)
  • Participate as a team member and attend scheduled staffings and supervision
  • Partner with clients and teams to identify treatment goals
  • Maintain clinical records per agency policy and procedure
  • Attend all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings
  • Manage all aspects of treatment, including referrals to additional resources when appropriate
  • Perform other duties as assigned
Requirements
  • 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positions
  • Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
  • Must possess a valid Driver License
  • Bachelor's degree in a related field or a combination of education and experience to equal 4 years in behavioral health will be considered
  • Minimum of a High School Diploma or GED required
  • Ability to assess situations and demonstrate critical thinking and problem-solving abilities
  • Understanding of mental health and its impact on functioning
  • Understanding of trauma-informed care
  • Experience working with individuals in crisis situations and/or in need of intensive support services
Working Conditions
  • Office environment, travel within the community as needed
  • Heavy exposure/use of computers, cell phones, office phones, etc.
  • Ability to sit for prolonged periods throughout work hours
  • Must be able to occasionally lift and/or move 5 or more pounds
  • Must be able to respond appropriately and effectively in crisis situations by using good judgment and following Polara Health protocols
