Caregiver Outreach Coordinator

Job Summary: Western Slope In-Home Care is seeking a dedicated professional to serve as a Field Supervisor Assistant. This role involves providing exceptional support to clients and caregivers in the field, ensuring the delivery of high-quality care services.

Key Responsibilities:

• Conduct regular field visits to assess client needs and caregiver performance
• Develop and implement care plans to meet client requirements
• Provide training and guidance to field staff on care delivery and software systems
• Manage client intake and scheduling, ensuring seamless service delivery
• Document and report changes in client status, service requirements, and staff performance
• Participate in the evaluation of field staff, focusing on punctuality, reliability, and quality of care

Requirements:

• High School Graduate or equivalent
• At least one year of experience in home/health services
• Excellent communication skills, with bilingualism in Spanish a plus
• Healthcare experience and/or software and billing experience in the healthcare field
