Job Summary
">We are seeking a skilled Pediatric Care Coordinator Specialist to join our professional team. The successful candidate will be responsible for coordinating early intervention services for families of children from birth to 3 years old, ensuring they receive necessary support and resources.
The Pediatric Care Coordinator Specialist will play a vital role in developing Individualized Family Service Plans (IFSPs), coordinating services, and monitoring progress towards established goals.
Key Responsibilities

- Assist and enable families to access early intervention services, including explaining the process, parent rights, and safeguards;
- Gather information from families regarding their concerns, priorities, and resources;
- Coordinate evaluation and assessment activities, facilitating eligibility determinations and connecting families with relevant resources;
- Organize and facilitate Transdisciplinary Team Consultation Meetings (TTCMs) and IFSPs, ensuring timely delivery of services;
- Maintain accurate records, documenting time spent with families and work done on their behalf;
- Enter documentation into the MAPS system and submit monthly billing submissions in accordance with program guidelines.
Requirements

- A bachelor's degree in social work, counseling, psychology, nursing, special education, or a closely related field;
- Proficient computer skills in Microsoft Windows, organizational, and record-keeping skills;
- Excellent oral and written communication, interpersonal, and teaming skills;
- Ability to train clients, family members, and professionals, maintain confidentiality, and adhere to MECA policies and procedures;
- Attending required Family Service Coordination training modules within one year of hire and obtaining certification through the NM FIT program.
