Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Primary Care team at Ochsner Health. As an LPN, you will play a vital role in providing direct patient care and working collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Provide direct care to assigned patients, including administering medications, performing vital signs, and documenting patient information.
  • Work collaboratively with the healthcare team to develop and implement individualized patient care plans.
  • Assist with patient education and support, promoting health literacy and empowering patients to take an active role in their care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
  • Maintain accurate and timely documentation of patient information, ensuring compliance with regulatory requirements.
Requirements
  • Current LPN licensure in the state of practice.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • IV therapy credentials (preferred).
  • Excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
