About Our Advanced Illness Management Program

Illumia Health, a part of Gentiva, an industry leader in hospice and palliative care, is seeking a skilled Registered Nurse (RN) to join our team as an Advanced Illness Management Nurse Specialist. This role offers a unique opportunity to make a difference in the lives of patients and their families facing advanced illnesses.

As an RN with Illumia, you will be part of a close-knit team that provides personalized care and support to patients in need. You will work collaboratively with healthcare professionals to implement patient-centered care plans, ensuring that patients receive the highest quality of life possible.

The ideal candidate will have 2 years of experience in skilled nursing, with a strong background in palliative care, oncology, hospice, or geriatric care. A current RN license to practice in the state and CPR certification are also required. Fluency in electronic medical record documentation and Microsoft Office software is essential for this role.

Responsibilities and Requirements
  • Triage patient needs over the phone and provide after-hours support as needed
  • Collaborate with the patient/family, attending physician, and other members of the IDG to provide patient-centered care
  • Maintain accurate clinical notes and communicate effectively with the AIM team and other stakeholders
  • Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess as necessary
