We are seeking a skilled and compassionate MDS/Enrollment Nurse to join our team at Uphams Corner Health Center. As a key member of our healthcare team, you will play a vital role in ensuring the clinical needs of our PACE participants are met.
Key Responsibilities- Conduct home visits to assess the clinical needs of prospective participants and provide personalized care plans.
- Collaborate with our interdisciplinary team to ensure seamless transitions and high-quality care.
- Manage all aspects of the clinical assessment process, including data entry and submission to regulatory agencies.
- Provide detailed clinical information to our IDTs to inform care decisions and ensure the best possible outcomes for our participants.
- Current MA Registered Nurse (RN) License and BLS certification.
- One year of experience working with an elder population, with a strong background in home care and clinical assessment.
- Excellent verbal and written communication skills, with the ability to work independently and as part of a team.
- Ability to identify ADL and Skilled Nursing needs, with a strong understanding of electronic health data systems.
- Experience with ICD-10 coding and electronic health data systems, including PACE CARE Online (PCO).
- Second language proficiency, with a strong preference for English/Spanish, English/Portuguese Creole, or English/French Creole.
