Hospice Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our hospice team. As a Hospice Nurse, you will provide holistic and personalized care to patients and their families during their end-of-life journey.
Key Responsibilities- Administer compassionate nursing care to hospice patients in accordance with their individualized care plans.
- Utilize the nursing process to assess patient needs, develop care plans, and implement appropriate interventions.
- Provide emotional support and guidance to patients and their families, fostering a comforting and supportive environment.
- Collaborate with an interdisciplinary team of healthcare professionals to ensure comprehensive and coordinated care.
- Document patient care accurately and timely, adhering to professional and regulatory standards.
- Current RN licensure with state of employment with no encumbrance.
- Previous nursing experience, preferably in hospice or palliative care, highly preferred.
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support to patients and families during a challenging time.
