Advanced Practice Provider

Responsibilities:
- Provide primary care to patients across the life span (family medicine) or adult patients (internal medicine).
- Collaborate with interdisciplinary health care team.
- Assess and diagnose patients, develop treatment plans, and provide preventive care.
- Document patient information in the EPIC system using SOAP format.
- Communicate with patients, patient care team, and staff.
Requirements:
- Master's Degree.
- National Board Certification as a Primary/Family Care Nurse Practitioner or Physician Assistant.
- BLS and ACLS certifications.
- Excellent written and verbal communication skills.
