About the Role

We are seeking a skilled and compassionate Licensed Physical Therapy Assistant to join our Home Health & Hospice team. As a Physical Therapy Assistant, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Implement and deliver physical therapy treatments to patients in their homes, under the supervision of a Physical Therapist.
  • Develop and maintain accurate patient records, utilizing mobile devices to ensure seamless communication and care coordination.
  • Collaborate with patients, families, and healthcare teams to provide comprehensive care and support.
  • Conduct thorough assessments and evaluations to identify patient needs and develop personalized care plans.
  • Stay up-to-date with industry developments and best practices, participating in ongoing education and training to enhance skills and knowledge.
Requirements
  • Associate degree in Physical Therapy Assistant Technology from a Commission on Accreditation in Physical Therapy Education (CAPTE) or equivalent accredited program.
  • Current unencumbered license for the state of Ohio to practice as a Physical Therapy Assistant (LPTA).
  • One year of experience as a Licensed Physical Therapy Assistant in a hospital, long-term care facility, or home health agency (new graduates welcome).
  • Valid driver's license, insurance, and acceptable driving record.
