Job Summary

We are seeking a skilled Life Skills Clinician to join our team at Community Health Network. As a Life Skills Clinician, you will work with individuals with mental health and/or substance use disorders, providing care coordination and life skills training and development.

Key Responsibilities
  • Collaborate with the care team to provide comprehensive psychosocial assessments and develop individualized treatment plans.
  • Provide life skills training and development to individuals, focusing on areas such as daily living skills, social skills, and coping mechanisms.
  • Document services provided in the Electronic Medical Record (EMR) and maintain accurate records.
  • Transport clients in personal vehicles when necessary.
Requirements
  • Bachelor's Degree in mental health or a related field.
  • Current Indiana Driver's License.
  • Ability to provide proof of auto insurance and have own reliable transportation.
  • Two years of social service experience in a volunteer or paid role (preferred).
