Director of Social Services

Wyndemere Senior Living Campus is seeking a Director of Social Services to coordinate and support all social services related functions within the Social Services Department. The ideal candidate will have a Bachelor's degree in Social Services or a related field and 3-5 years of experience working in a skilled nursing facility.

Key Responsibilities:
  • Assess, plan, implement, and evaluate the social, emotional, and psychological needs of sub-acute patients.
  • Coordinate all Health Center admissions and discharges through review with patients and families.
  • Identify and assist with mood, behavior, and psychosocial well-being issues for residents.
  • Provide ongoing communication with family/patient regarding discharge needs.
  • Utilize problem-solving skills to assist patient and family in identifying potential barriers to discharge.
Requirements:
  • Bachelor's degree in Social Services or related field.
  • 3-5 years of experience working in a skilled nursing facility.
  • Knowledge of Federal and State regulations for skilled nursing facilities.
  • Comprehensive understanding of the English Language.

Salary: $60,000
