Ambulatory Care Registered Nurse

We are seeking a highly skilled Ambulatory Care Registered Nurse to join our team at Mass General Brigham. As a Primary Care RN, you will play a critical role in providing high-quality patient care and contributing to the success of our multi-disciplinary team.
Responsibilities will include:
- Providing culturally sensitive, patient-centered care to patients of all backgrounds and income levels
- Triage and assessment of patient care needs, including routine and chronic illnesses
- Developing and implementing individualized nursing plans of care (PoC)
- Collaborating with the clinical team and family to promote self-management and ensure a cycle of continuous refinement of PoC
- Ensuring effective coordination of care across the care continuum
Requirements:
- Graduate of an approved school of nursing with current registration in Massachusetts
- Bachelor of Science Degree in Nursing is required for newly licensed nurses
- Bilingual Spanish preferred
- Recent (within 3 years) Ambulatory Care experience required
- Triage experience preferred in Primary Care, Urgent Care, or Community Health nursing
