Position Overview: We are seeking a dedicated Hospice RN Case Manager to provide exceptional care during critical times. Our mission is to deliver personal, palliative, and hospice care that respects the dignity and wishes of our patients.

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, overseeing the management of each patient’s care.
  • Serve as the key point of contact among the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients while offering supportive assistance to their families, maintaining a manageable caseload to foster trust and compassion.
  • Empower caregivers by providing education on effective care practices for their loved ones.
  • Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for our patients.
  • Assess patient and family needs, aiding in the development and implementation of personalized care plans.
  • Maintain comprehensive clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives within the organization.

Candidate Profile: Required Qualifications:

  • A genuine hospice heart: compassionate, kind, and empathetic towards patients and their families.
  • Ability to provide care in various settings as needed.
  • Exhibit flexibility and composure while supporting individuals during challenging times.
  • Valid RN license in the state of employment (or eligibility to obtain).
  • Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in areas such as hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is advantageous.
  • Current driver's license and automobile insurance are required.
  • Current CPR certification is mandatory.
