Job Summary

A-Line Staffing Solutions is seeking a highly skilled and experienced Lead Social Worker to join our team in Dayton, OH. As a key member of our clinical team, you will be responsible for providing short-term therapeutic counseling and crisis intervention to patients and families with psycho-social needs.

Key Responsibilities
  • Conduct psychosocial assessments and develop, update, and maintain the Care/Treatment Plan based on facility policy, procedures, Joint Commission standards, and state/federal regulations.
  • Work closely with the health care team to develop and coordinate an individualized discharge plan for patients, ensuring a smooth transition to aftercare.
  • Provide group and family process-oriented therapies, as well as educational and didactic groups for patients and family members.
  • Communicate with physicians to report patient progress, needs, concerns, and potential and actual problems.
  • Act as a resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
Requirements
  • Master's Degree in Social Work.
  • 3-5 years of experience as a social worker, with mental health experience given priority.
  • Licensed Independent Social Worker.
