Job Summary:The Certified Home Health Aide provides personal care and related services to patients in their homes, assisting with daily living activities and promoting independence and well-being. This role requires a compassionate and caring individual who is committed to delivering exceptional patient care.
Responsibilities:- Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
- Assist patients with mobility and transfer needs, including walking, transferring, and positioning
- Prepare and serve meals and snacks, as needed
- Assist with light housekeeping and laundry
- Provide emotional support and companionship to patients and their families
- Document patient care and progress in accordance with company policies and procedures
- High School Diploma or equivalent required
- 1 year of experience as a Hospice Aide or Nursing Assistant required
- State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
- Valid driver's license and insurance and reliable transportation to perform job tasks
- 50% travel
