Job Summary

The Licensed Clinician is a key member of our healthcare team, responsible for assessing, counseling, intervening, implementing, and educating patients, families, and significant others to facilitate movement throughout the continuum of care. This role requires a strong understanding of psychosocial assessments, counseling interventions, and case management services.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized plans of care.
  • Provide supportive counseling to patients and families, utilizing evidence-based interventions to enhance coping skills and promote positive outcomes.
  • Coordinate case management services, collaborating with patients, families, and healthcare teams to ensure seamless transitions and efficient resource utilization.
  • Develop and maintain good working relationships with healthcare and social service agencies to facilitate access to community resources.
  • Document comprehensive assessment findings and plans of care in the medical record, ensuring timely and accurate documentation.

Requirements

  • Master's degree in Counseling, Social Work, or a related field.
  • 2-4 years of experience in a healthcare setting, with a focus on psychosocial assessments and counseling interventions.
  • NJ LSW required; NJ LCADC preferred for addiction programs.
  • Valid NJ driver's license required for programs involving in-home visits.
