Job Summary
The Licensed Clinician is a key member of our healthcare team, responsible for assessing, counseling, and educating patients and their families to facilitate movement throughout the continuum of care.
Key Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
- Provide supportive counseling to patients and families, utilizing evidence-based interventions and group process to enhance coping skills.
- Coordinate case management services, collaborating with patients, families, and healthcare teams to ensure seamless transitions and efficient resource utilization.
- Document comprehensive assessment findings and care plans in the medical record, ensuring accurate and timely documentation.
Requirements
- Master's degree in Counseling, Social Work, or related field.
- 2-4 years of experience in healthcare, with a focus on patient care and case management.
- NJ LSW required; NJ LCADC preferred for addiction programs.
- Valid NJ driver's license for programs with in-home visits.
