Job Title: Care Navigator

We are seeking a highly skilled and compassionate Care Navigator to join our Outpatient Competency Restoration team at AllHealth Network. As a key member of our team, you will play a vital role in providing competency restoration education services to individuals involved in the criminal justice system.

Job Summary:

The Care Navigator will be responsible for managing client caseloads, communicating with care providers, and tracking inactive caseloads for clinical providers. This role requires excellent customer service skills, strong organizational abilities, and flexibility in responding to multiple demands.

Key Responsibilities:
  • Extract and analyze Excel-based data reports from the electronic health record on a weekly basis.
  • Develop and implement tracking mechanisms for interventions related to caseload management.
  • Communicate effectively with Central Access, clinicians, medical providers, supervisors, and managers regarding client needs and caseload status.
  • Complete administrative updates and closures of client charts based on clinical team direction and AllHealth Network policies.
  • Schedule and cancel client appointments based on case management reports and client communication.
  • Participate in telephonic communication with clients regarding their engagement in treatment and document all interventions in the electronic health record.
  • Assist with care coordination tasks, such as appointment monitoring and facilitating communication with providers and other care team members.
Requirements:
  • Bachelor's degree in a Social or Human Services field.
  • One year of experience in a customer service or healthcare-related field, preferably in a hospital, home health provider, or community-based agency.
  • Excellent customer service skills, strong organizational abilities, and flexibility in responding to multiple demands.
  • Ability to operate PC-based software programs, including high-level proficiency in Word, Excel, and other computer-based systems.
  • Understanding or willingness to learn care coordination and case management-related service functions.
