Job Description:

The Homecare RN Case Manager plays a vital role in the interdisciplinary team, ensuring seamless coordination of patient care services. This position requires strong leadership skills, effective communication, and a commitment to delivering high-quality patient outcomes.

Main Responsibilities:
  • Admission and discharge coordination
  • Interdisciplinary plan of care communication
  • Patient and family education/relations
  • Payor relations
  • Coordination of care for all disciplines and total fiscal management

The Homecare RN Case Manager is responsible for performing ongoing utilization and quality reviews, ensuring cost-effective treatment is provided by the team. This role also involves leadership responsibilities, including scheduling, staff competencies, new employee selection/orientation, constructive discipline, and shared leadership council representation.

Requirements:
  • Current RN license in state of practice
  • Current Driver's License in the state of practice
  • Current Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position
  • Basic Life Support (BLS)
  • Two years of clinical nursing experience
  • One year of experience working in Home Health or Hospice setting
  • Basic computer skills and knowledge of word processing software
  • Excellent written and verbal communication skills
  • Independent worker, who is self-motivated, has a positive attitude, and can be flexible in a rapidly changing healthcare environment

Preferred Qualifications:

  • Bachelor's degree in Nursing (BSN) from an accredited institution
  • Three years of experience working as a case/care manager in a home health or hospice setting
  • Experience working with third party payers
  • Knowledge of home-based health services and resources

Physical Requirements:

  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy
  • Expected to lift and utilize full range of movement to transfer patients
  • Frequently walks and assists with transporting/ambulating patients and obtaining and distributing supplies and equipment
  • Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles
