Role Overview:
The Social Services Assistant plays a vital role in supporting the social service team by offering administrative assistance throughout the patient journey from admission to discharge.
Key Responsibilities:
*Engage with patients/residents and their families to provide essential support and resource information.
*Address concerns raised by patients/residents and their families.
*Assist in the transfer of patients/residents within the facility to ensure smooth transitions and adjustments, documenting the process accordingly.
*Contribute to the healthcare decision-making process within the facility under the guidance of a qualified social worker or consultant.
