Job Title: RN Field Case Manager

We are seeking a compassionate and experienced Registered Nurse to join our team as a Field Case Manager. As a Field Case Manager, you will provide skilled nursing care to patients in their homes, working closely with our clinical team to ensure the highest level of care and support.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, infusion therapy, and other specialized services.
  • Conduct initial evaluations and regularly reevaluate patient needs to ensure the most effective care plan.
  • Initiate and revise care plans as needed, working closely with our clinical team and patients to ensure the best possible outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and care plans in a timely and accurate manner.

Requirements:

  • Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
  • One year of nursing experience, with a focus on home health care and skilled nursing services.
  • Experience with Kinnser EMR and other electronic medical records systems.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
