About the Role

We are seeking a skilled Registered Nurse to join our team at Almost Family, a part of LHC Group. As a Registered Nurse in Home and Community Based Services, you will provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.

Key Responsibilities
  • Provide clinical nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
  • Ensure patient eligibility and medical necessity for services, and initiate plan of care revisions as needed with physician approval.
  • Collaborate with other community providers to develop and revise individualized plans of care and service plans.
  • Ensure plan of care frequency and duration meets the needs of the patient.
Requirements
  • A minimum of one year experience as an RN is preferred.
  • Current RN licensure in the state of practice is required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
  • Current CPR certification is required.
