Job Description

The Integrated Transitional Care Nurse (RN) plays a critical role in ensuring seamless transitions between care settings, collaborating with hospitals and Independent Physician Associations to deliver continuous, quality healthcare to high-risk members. Key responsibilities include conducting discharge risk assessments, overseeing reviews, and ensuring compliance with regulatory requirements.

Required Skills and Qualifications

To be successful in this role, you will need:

  • Two or more years of utilization management/case management experience in a healthcare delivery setting, with an emphasis on concurrent review and utilization management
  • Possession of an active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN
  • Valid California Driver's License and valid automobile insurance
About the Role

This position is based in Rancho Cucamonga, CA, and requires working on a hybrid schedule, with Monday and Friday remote workdays and Tuesday through Thursday onsite at our office. The ideal candidate will have strong analytical skills, excellent communication and interpersonal skills, and the ability to work effectively in a team environment.
