About the Position
This is a full-time position that requires a high level of autonomy and self-motivation. As a Client Advocate III - Permanent Supportive Housing, you will work closely with clients to assess their needs, develop personalized service plans, and connect them with community resources. Your strong communication and organizational skills will enable you to build effective relationships with clients, colleagues, and community partners. The estimated salary for this position is $70,000-$80,000 per year, depending on experience.
Key Responsibilities
Your key responsibilities will include:
- Providing comprehensive case management services to clients, including intake, assessment, goal setting, monitoring, and reassessment
- Developing and maintaining relationships with community partners to access housing, employment, and other essential services
- Maintaining accurate and timely records of activities and services provided to clients in the Homeless Management Information System (HMIS) database system, case management notes, and other agency tracking tools
