Key Responsibilities- Certification Requirements: Must possess BLS certification; Case Management certification is preferred.
- Job Role: The Nurse Case Manager I will report directly to the Manager of Case Management and Care Transitions, working collaboratively with the clinical team.
- Patient Ratios: Manage a patient ratio of 1:30.
- Work Schedule: This role involves working five 8-hour shifts from 8:00 AM to 4:30 PM, with flexibility for holidays as necessary.
- Weekend Commitment: Required to work two weekend shifts per contract.
- On-call Duties: No on-call responsibilities are expected.
- Clinical Expertise: Act as a clinical expert in discharge planning and resource utilization.
- Experience: A minimum of three years of RN experience is required, with travel experience being essential.
- Education: A Bachelor of Science in Nursing (BSN) is mandatory.
- Preferred Skills: Prior leadership experience and familiarity with Epic systems are advantageous.
- Shift Details: Standard shifts are five 8-hour days, with two weekend shifts required.
- Block Scheduling: Open to block scheduling with prior approval, though not guaranteed.
- Time Off Requests: All requests for time off must be submitted at the time of application.
- Local Candidates: Consideration for local candidates is available on a case-by-case basis.
- Licensing: A valid SC or compact state license is required at the start of employment.
