Job Title: Inpatient Rehab Registered Nurse

HCA South Atlantic Division is seeking a highly skilled Registered Nurse (RN) to join our team as an Inpatient Rehab Case Manager. This is an exciting opportunity to make a real impact in the lives of our patients and contribute to the advancement of nursing practice.

Job Summary:

We are looking for a dedicated and compassionate RN to provide clinical expertise in coordinating patient care, managing length of stay, and ensuring appropriate resource management. As an Inpatient Rehab Case Manager, you will be responsible for developing safe and effective discharge plans, facilitating patient progression and transition of care, and identifying patients at risk for adverse outcomes.

Responsibilities:
  • Coordinate patient care plans, manage length of stay, and ensure appropriate resource management
  • Develop safe and effective discharge plans, considering patient needs and resources
  • Facilitate patient progression and transition of care, identifying patients at risk for adverse outcomes
  • Perform comprehensive assessments of patient needs, including psychosocial, medical, and discharge needs
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
Requirements:
  • Graduate from an accredited school of nursing (Associate's Degree or higher)
  • Licensed as a Registered Nurse in the state of practice (Compact/Multi-State RN license accepted)
  • 1-3 years of clinical, hospital nursing experience (2 years of case management experience preferred)
