Job Description:

We are seeking a skilled Speech Language Pathologist to join our team at Alternate Solutions Health Network LLC. As a Speech Therapist, you will provide home-based skilled speech therapy care for patients as directed by the attending physician.

Responsibilities:

  1. Develop an individualized ST plan of care and make adjustments as needs change.
  2. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.
  3. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
  4. Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
  5. Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  6. Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
  7. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  8. Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  9. Attend in-service training and mandatory agency meetings.

Requirements:

  • Speech Therapist with a current license in the state of employment.
  • Minimum one-year experience as a ST in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
