As a Practical Nursing Specialist, your responsibilities will include:
- Delivering in-home direct patient care according to the prescribed plan of care while closely monitoring progress and recovery.
- Evaluating and documenting patient conditions and reporting changes as necessary (vitals, blood/stool/urine, glucose, etc.).
- Administering medications, insulin, and IV fluids while ensuring thorough documentation.
- Inspecting and caring for wounds, changing dressings, and assisting with personal hygiene.
- Assessing patients for further injuries, infections, bedsores, etc., and providing appropriate treatments.
- Collaborating with healthcare providers, aides, and allied health professionals to ensure exceptional patient outcomes.
To qualify for a Practical Nursing Specialist position with us, you should have:
- Education: Completion of a practical (vocational) nursing program for LPN/LVN and possess a current license as an LPN/LVN in the relevant state(s) with at least 1200 hours of experience or completion of the Office's preceptor program.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to and from care sites and/or work locations.
- Experience with trach and/or ventilators is preferred but not mandatory.
