The Clinical Transition Nurse (CTN) plays a crucial role in ensuring the smooth and secure transition of patients from acute care settings to various alternative care options. These may include home care services, palliative or hospice care, skilled nursing facilities, or other suitable environments tailored to meet the individual needs of patients.
Key Responsibilities:
- Facilitate essential activities related to patient discharge.
- Support the admissions process and other operations to enhance patient flow.
Qualifications:
- Current, unrestricted LPN license in Alabama.
- A minimum of three years of nursing experience in a medical-surgical environment.
Language and Communication Skills:
- Proficient in reading, writing, and speaking English.
- Ability to effectively communicate with individuals from diverse educational and cultural backgrounds, both verbally and in writing.
- Exceptional customer service and interpersonal skills.
Additional Skills:
- Completion of annual competency requirements.
- Experience with personal computers and software applications in a Windows environment.
- Capability to work independently while being an integral part of the healthcare team.
- Strong conflict resolution, problem-solving, leadership, and organizational skills, along with proficient written and verbal communication abilities.
Work Schedule:
Day shift from 8:00 AM to 4:30 PM.

Regular, Full-Time.
