Key Responsibilities
  • Provide personalized care and support to senior clients, including assistance with activities of daily living, medication management, and companionship.
  • Work collaboratively with the On Call Supervisor to ensure seamless care delivery and respond to urgent client needs.
  • Communicate effectively with clients, families, and healthcare professionals to ensure comprehensive care.
  • Maintain accurate records and documentation of client care, including progress notes and medication administration.
  • Participate in ongoing training and education to enhance skills and knowledge in geriatric care.
Requirements
  • Experience working with elderly clients, preferably in a home health care setting.
  • Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Ability to lift, transfer, and position clients with care and safety.
  • Valid driver's license and reliable transportation.
