- Work collaboratively with the On Call Supervisor to ensure seamless care delivery and respond to urgent client needs.
- Communicate effectively with clients, families, and healthcare professionals to ensure comprehensive care.
- Maintain accurate records and documentation of client care, including progress notes and medication administration.
- Participate in ongoing training and education to enhance skills and knowledge in geriatric care.
- Experience working with elderly clients, preferably in a home health care setting.
- Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
