Home Health Clinician

We invite applications from experienced healthcare professionals seeking a challenging yet rewarding role as a Home Health Clinician with our HouseCalls team in Oregon. In this position, you will play a vital role in providing annual clinical assessments to patients in their homes, identifying health risks, and coordinating follow-up care.
The successful candidate will possess excellent communication skills, a commitment to patient-centered care, and a passion for delivering high-quality services. As a key member of our integrated medical team, you will work collaboratively with other healthcare professionals to promote seamless care transitions and enhance patient well-being.
A competitive salary of $95,000 - $125,000 per year is offered, along with a $40,000 student loan repayment program for eligible candidates.
Responsibilities:
- Conduct comprehensive home assessments, including reviews of past medical history, medication lists, and vital signs.
- Perform evidence-based practice screenings and point-of-care testing (as necessary).
- Develop care management plans to address health gaps and promote active medical management.
- Collaborate with primary care physicians to inform care decisions and coordinate follow-up care.
- Respond to urgent and emergent situations, ensuring patient safety and well-being.
Requirements:
- Active NP or PA license or ability to obtain by start date, with unencumbered licensure.
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or equivalent.
- Ability to complete physical requirements of the job, including lifting 30-pound bags and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
