Position Overview:


Delivers physical therapy services to individuals experiencing impairments, functional limitations, disabilities, or alterations in physical health due to injury, illness, or other factors, under occasional supervision.

Engages in collaborative efforts with the healthcare and educational teams to achieve patient care objectives, outcomes, and systemic enhancements.

PRIMARY RESPONSIBILITIES

  • Evaluates comprehensive data from both formal and informal assessments relevant to patient care and management to establish a rehabilitation treatment diagnosis.
  • Works alongside stakeholders to formulate the care plan, coordinates and executes care delivery while anticipating patient needs throughout the rehabilitation process.
  • Implements suitable, evidence-based interventions to guarantee effective outcomes and regularly reassesses progress towards established goals.
  • Utilizes strategies to facilitate a successful transition back to home, community, and educational/vocational settings.
  • The responsibilities outlined are a general overview of the role, and additional tasks may be assigned as necessary.

TECHNICAL SKILLS

  • Treatment Planning & Goal Setting (Novice): Exhibits foundational proficiency in crafting a care plan that integrates treatment interventions aimed at achieving specific objectives. Develops and documents patient-centered short and long-term goals that are functional and measurable. Recognizes the need for and refers to additional services as required.
  • Rehabilitation Therapist Patient Evaluation (Novice): Demonstrates capability in assessing patients with physical deficits, including impairments, developmental disabilities, functional limitations, or changes in physical health by gathering relevant information and administering standardized tests and objective measures. Selects appropriate assessment tools and conducts functional evaluations to document patient status requiring skilled therapy. Interprets assessment results and their implications for the patient, family, referring physician, and other healthcare team members.
  • Treatment Implementation (Novice): Shows proficiency in delivering treatment aligned with physician directives and the established care plan. Able to adjust treatment plans as necessary to ensure the provision of skilled therapy services, including the progression of treatment throughout the care continuum and revision of interventions for unmet goals. May oversee and guide certified athletic trainers (PT only), physical therapist/occupational therapist assistants, technicians, volunteers, and students. Adheres to established protocols for care and employs evidence-based treatments. Has completed a minimum of one year of clinical experience in treatment implementation.
  • Rehab Therapist Patient Documentation (Novice): Demonstrates proficiency in documentation practices. Utilizes appropriate templates and approved abbreviations within patient records. Meets institutional standards for documentation quality and timeliness.
  • Physical/Occupational Therapist Patient Education (Novice): Exhibits proficiency in conveying the functional implications of assessment findings, treatment interventions, treatment planning diagnoses, and disease processes. Considers the communication needs and comprehension levels of the audience and may share relevant information/literature. May supervise, educate, and train students from professional programs in physical therapy, occupational therapy, and athletic training.
