Home Health RN Case Manager

The RN Case Manager at Pathwell Health is responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining accurate medical records.
Key responsibilities include:
- Providing skilled nursing treatments as ordered by the client's doctor
- Completing evaluation tasks, including vital signs and medication reviews
- Coordinating with physicians, therapists, and other individuals in the client's care plan
- Assessing and charting observations of the client's condition at each visit
- Maintaining complete and accurate documentation of nursing care
- Working collaboratively with all members of the healthcare team
- Providing education to clients and families on proper home health care procedures and strategies
- Recording patients' medical information and vital signs
- Monitoring, recording, and reporting symptoms or changes in patient's conditions
- Administering local, inhalation, intravenous, or other anesthetic
- Preparing patients for and assisting with examinations or treatments
- Modifying patient treatment plans as indicated by patients' responses and conditions
- Ordering, interpreting, and evaluating diagnostic tests to identify and assess the patient's condition
- Monitoring all aspects of patient care, including diet and physical activity
- Engaging in research activities and participation in formal educational training activities related to nursing
- Performing other duties as assigned
Requirements include:
- Associate degree required; Graduate of an accredited state-approved, Bachelor's Degree from the school of nursing preferred
- Current and valid Connecticut Registered Nurse License
- Prior nursing experience, specifically in home health (preferred)
- Current CPR certification (required)
- Driver's License, Car Insurance, Car Registration
- Strong interpersonal skills that build personal rapport and establish trust with the client
- Computer literate
- Strong organizational skills
- Excellent written and communication skills
- Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
- Home health care experience
- Bilingual (Spanish) preferred
