LPN Home Health Job Description

Sigma Healthcare is seeking a skilled LPN to provide high-quality care to patients in their homes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including administering medications and treatments as ordered by physicians.
  • Assess and document patients' condition, notifying healthcare professionals of any significant changes.
  • Develop and implement care plans in collaboration with the healthcare team.
  • Educate patients and their families on disease management, self-care, and medication administration.
  • Monitor and evaluate the effectiveness of care provided, ensuring compliance with regulatory standards and policies.
  • Maintain accurate and complete medical records.
  • Collaborate with other healthcare professionals to coordinate services and facilitate smooth transitions of care.
  • Provide emotional support and compassionate care to patients and their families.
Requirements:
  • Must be a licensed practical nurse (LPN) or licensed vocational nurse (LVN) with a valid license in the state of practice.
  • Graduation from an accredited practical nursing program.
  • Prior experience in home health or a related field is preferred.
  • Excellent clinical and critical thinking skills.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Knowledge of medical terminology and patient care procedures.
  • Basic computer skills for documentation and communication purposes.
  • Physical stamina and the ability to lift and move patients when necessary.
  • Valid driver's license and reliable transportation as travel to patients' homes may be required.
