We are seeking a skilled RN Diabetes Educator to join our team as a Diabetes Program Manager. The successful candidate will be responsible for planning, implementing, and evaluating our Diabetes Services program. This role requires a strong background in diabetes care and education, as well as excellent communication and leadership skills.
Responsibilities- Plan and Implement Diabetes Services Program: Develop and implement a comprehensive diabetes services program that meets the needs of our patients and community.
- Provide Education and Training: Provide education and training to diabetic inpatients and outpatients, as well as staff education related to diabetes.
- Maintain Program Recognition Status: Maintain the program's ADA recognition status, if applicable.
- Collaborate with Healthcare Team: Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Education: Bachelor of Science Degree in Nursing preferred.
- Experience: Minimum 2 years experience as an RN, with experience in program management in a health-related field and care of people with diabetes.
- Certifications: Certified Diabetes Educator (CDE) strongly preferred.
- Language Skills: Bilingual English/Spanish strongly preferred.
- Communication Skills: Clear, professional, personable communication skills.
