Job Summary:

The Social Worker provides support and assistance to residents and their families, addressing personal and environmental challenges that may impact their well-being.

Key Responsibilities:

  • Assess and address resident needs, collaborating with healthcare teams to ensure comprehensive care.
  • Develop and implement individualized plans to promote resident independence and quality of life.
  • Provide emotional support and counseling to residents and their families, addressing concerns and issues related to illness, disability, and aging.
  • Maintain accurate records and documentation, ensuring compliance with regulatory requirements.
  • Collaborate with community resources and agencies to facilitate resident discharge and transition to outpatient settings.

Requirements:

  • Bachelor's degree in Social Work or related field required.
  • Minimum of two years of experience in a Social Work Department health care setting required.
  • Skilled Nursing experience preferred.
  • Valid PA Driver's License required for travel within community.
  • PA license to practice Social Work is preferred.

Working Conditions:

The Social Worker works in a well-lit office environment and other hospital departments, requiring the ability to transport themselves throughout the facility.
