Job Summary

Elevate patient care and outcomes as a skilled RN Patient Placement Specialist at Prisma Health. This role requires a unique blend of clinical expertise, communication skills, and business acumen to coordinate seamless transitions between healthcare settings.

Key Responsibilities
  • Patient Assessment: Conduct thorough assessments to identify patient needs and appropriateness for post-acute services, communicating results to patients, physicians, and multidisciplinary teams.
  • Planning: Collaborate with patients, families, and healthcare teams to develop personalized placement plans, ensuring timely and effective transitions.
  • Implementation: Facilitate patient education, coordinate discharge planning, and ensure compliance with regulatory requirements, while maintaining open communication with stakeholders.
  • Program Tracking and Evaluation: Monitor and analyze clinical and fiscal outcomes, presenting data to management and participating in program evaluation and quality improvement initiatives.
  • Leadership: Serve as a clinical resource and mentor, providing education and training to new employees, and actively seeking opportunities for continuing education and professional development.
Requirements
  • Associate's Degree in Nursing
  • 2+ years of post-acute related experience
  • Current RN licensure
  • Bachelor's of Science in Nursing (BSN) or Certified Rehabilitation Registered Nurse (CRRN) preferred
