Personal Care Assistant

Certification Requirement: Must hold a valid nursing assistant certification in North Carolina.
Overview:
As integral members of the healthcare team, our aides deliver essential personal care, monitor changes in the patient's or client's health status, execute basic procedures under the guidance of nursing or therapy services, and assist with daily living activities as designated for clients in their homes, adhering to the standards set by Interim HealthCare and applicable regulations.
Key Responsibilities:
- Monitor and document vital signs including temperature, pulse, and respiration rates.
- Assist clients and their families with daily living tasks such as bathing, grooming, skin care, oral hygiene, and nutritional support.
- Ensure a safe environment by maintaining clear pathways during care provision.
- Support clients with mobility, transfers, and range of motion exercises.
- Help with incidental activities such as shopping, meal preparation, social interactions, and maintaining a tidy living space.
- Provide reminders for medication schedules.
- Assist in the administration of medications that clients typically manage themselves.
- Implement infection control measures, including hand hygiene and the use of personal protective equipment.
- Adhere strictly to the care plan, performing only the tasks assigned.
- Identify and report any changes in the client's condition to the supervisor.
- Document all observations, activities, and services provided accurately and promptly.
- Organize and manage time effectively to complete visits and tasks.
- Participate in mandatory training sessions and maintain up-to-date employee records.
- Engage in quality improvement initiatives as requested.
- Contribute to emergency preparedness efforts as needed.
- Complete additional tasks as assigned.
- Respect and uphold patient rights.
- Access necessary personal health information (PHI) in accordance with organizational policies.
Prohibited Actions:
- Perform any skilled nursing tasks that are restricted by state regulations.
Education & Experience Requirements:
- Must be at least 18 years old.
- Minimum of six months experience in a healthcare setting or completion of a preceptor program.
- Must be a nursing student in the final year of training or possess documentation of competency in personal care and home health aide skills, or be a graduate RN or LPN/LVN.
- Must have completed a state-approved aide training and competency evaluation program.
- No gaps in compensated work as a home health aide or nurse aide for the past 24 months; otherwise, re-training is required.
Knowledge, Skills & Abilities:
- Two positive professional references.
- Successful completion of a written knowledge examination for home health aides.
- Pass state-mandated criminal and abuse background checks.
- Effective communication skills in English, both verbal and written.
- Able to prioritize and manage multiple tasks efficiently.
- Must provide proof of a valid driver's license and auto liability insurance if applicable.
Work Environment & Physical Demands:
- Frequent travel within the service area for client visits.
- Must meet health requirements for patient care as per applicable laws.
- Work primarily takes place in clients' homes.
- Potential exposure to communicable diseases and hazardous materials; adherence to safety protocols is essential.
- Ability to stand, bend, stoop, kneel, and reach frequently.
- Must be able to lift, reposition, and transfer clients as needed.
- Physical activity may include lifting and carrying items up to 50 lbs.
