At Islands Hospice, we are seeking a skilled Nursing Care Coordinator to join our team. As a key member of our interdisciplinary group, you will play a vital role in providing high-quality patient care and coordinating services to meet the needs of individuals and families within their homes and communities.

Job Summary:

The Nursing Care Coordinator is responsible for planning, organizing, and directing hospice care, with a focus on community health education and experience. This role requires strong nursing skills, excellent communication abilities, and the ability to work effectively with patients, families, and healthcare professionals.

We offer a competitive salary range of $80,000.00 - $101,000.00 per year, along with a comprehensive benefits package that includes 401(k), dental insurance, health insurance, paid time off, and vision insurance.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patients and families to determine hospice needs and develop individualized care plans
  • Collaborate with patients, families, and healthcare professionals to coordinate care and ensure continuity of services
  • Provide education and support to patients and families on self-care techniques, medication management, and other aspects of palliative care
  • Communicate effectively with physicians, nurses, and other healthcare professionals to ensure seamless coordination of care
  • Supervise hospice aides and provide guidance and support as needed
Requirements:
  • Graduate of an accredited school of nursing with at least one (1) to two (2) years of recent acute care experience in an institutional setting
  • Current licensure in the state and CPR certification required
  • Must be a licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working order
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist
