Seeking a highly skilled Licensed Vocational Nurse to join our team at Alameda Health System. As a key member of our healthcare team, you will provide exceptional patient care and support in our FQ MCH/OB Clinic (K6).

About the Job:

This is a full-time, day shift position that offers a competitive salary range of $68,000 - $92,000 per year, depending on experience. Our ideal candidate will be a compassionate and detail-oriented Licensed Vocational Nurse with excellent communication skills and a passion for delivering high-quality patient care.

Job Summary:

We are seeking an experienced LVN to provide comprehensive nursing care to patients in our clinic setting. Your responsibilities will include administering medications and immunizations, performing treatments, supporting team-based care delivery, and assisting medical personnel with procedures. You will also be responsible for educating patients and their families on health-related topics and providing emotional support during challenging times.

Key Responsibilities:

  • Provide patient-centered care that is respectful of, and responsive to, individual patient preferences, needs, and values.
  • Collaborate with patients and other healthcare providers to deliver coordinated, high-quality care.
  • Prepare and administer medications, determine immunizations needed, and give injections.
  • Maintain current knowledge/certification/licensure per AHS policy and pursue professional growth and development.
  • Collect information, maintain up-to-date and accurate documentation of nursing care provided, objective data (e.g., vital signs), subjective information (e.g., patient history) to ensure the integration of information for use by the healthcare team, to ensure quality care.
  • Teach and counsel patients on a variety of health-related subjects.
  • Monitor patient flow and assign/direct other ancillary personnel as needed.
  • Effectively utilize population health methodologies such as patient registry, in-reach, outreach, care protocols, and team-based care to improve patient and population outcomes.
  • Perform routine care and cleaning of patient care equipment.
  • Perform point-of-care testing and draw blood samples from patients.
  • Keep accurate patient records.
  • Respond effectively to emergency situations.
  • Reviews patient record to note condition of patients and treatments; medications, history, and procedures ordered by physicians.

Requirements:

  • BLS - Basic Life Support Certification issued by the American Heart Association.
  • Valid license to practice as a Licensed Vocational Nurse in the State of California.
  • Graduate of an accredited LVN program.
  • Minimum experience: Graduate from a school of vocational nursing within the last twelve months prior to hire date, or have had recent vocational nursing refresher courses, or have had recent related experience in an acute/outpatient setting.
