Key Responsibilities:
As a Resident Care Coordinator, you will:
- Establish objectives aimed at enhancing treatment and care strategies within the facility.
- Assess the quality of care provided to residents.
- Collaborate with nursing personnel, caregivers, and family members to review health conditions and care plans.
- Authorize resident admissions to the facility.
Qualifications:
- Bachelor’s degree in Nursing.
- Valid nursing license in Illinois.
- Minimum of 2 years’ experience in a long-term care setting.
- Prior experience in a Resident Care Coordinator position is preferred.
- Strong communication and organizational abilities.
