Job Summary

The Registered Nurse Discharge Planner is responsible for performing case management, utilization review, quality assurance, and discharge planning. This role requires a strong understanding of patient needs and the ability to coordinate care across multiple settings.

Responsibilities
  • Assess patient needs and develop individualized discharge plans
  • Coordinate with interdisciplinary teams to ensure seamless transitions of care
  • Perform utilization review and quality assurance activities to ensure compliance with regulatory requirements
  • Develop and maintain relationships with community resources and providers to ensure access to necessary services
  • Provide education and support to patients and families to promote self-management and independence
  • Collaborate with healthcare teams to identify and address barriers to discharge
  • Document all activities and outcomes in the patient's medical record
  • Participate in ongoing quality improvement activities to enhance the discharge planning process
Requirements
  • Current RN licensure in the state of California
  • Graduate of an accredited school of professional nursing
  • Ability to interact therapeutically with patients, families, physicians, co-workers, and community agencies
  • Ability to maintain confidentiality
  • High level organizational skills
  • Excellent communication skills
  • Current American Heart Basic Life Support (BLS) certification
